Comprehensive Perinatal Services Program Health Worker (CPHW)

Full Time Position: Comprehensive Perinatal Services Program Health Worker (CPHW)

Location: CHC Women Health Clinics in Templeton and San Luis Obispo

SUMMARY:

Under the direct supervision of the CPSP Coordinator, the CPHW works as part of a team to provide efficient and quality patient perinatal care. The employee is expected to be cooperative and share an enthusiastic attitude. It is the primary purpose of CHCCC to provide the highest quality of health care possible to the population it serves. Such a level of quality depends ultimately on the staff’s desire and ability to work together.

RESPONSIBILITIES:

  • Helps to ensure that all CPSP Health Education, Nutrition and Psychosocial services delivered to clients are appropriate for their needs. It involves organizing the provision of comprehensive perinatal services and includes, but is not limited to, supervision of all aspects of client care including antepartum, intrapartum and postpartum. .
  • Works closely with members of the health team and the client to develop and implement the Individualized Care Plan (ICP).
  • Complete patient orientations, initial and re-assessments based on client needs in areas of nutrition, psychosocial and health education per CPSP guidelines.
  • Provide culturally sensitive health education materials, including aids suitable for the client based on each client’s education care plan.
  • Assist the client with practical arrangements such as: transportation, translation needs, tests, referrals and special appointments.
  • Modify the individual care plan as the client condition changes.
  • Act as a liaison between the client and the health care team to promote effective communication.
  • Maintain close contact with the client throughout pregnancy and the postpartum period.
  • Track client’s attendance of appointments, identify missed appointments and assist to reschedule.
  • Participate in community outreach events as needed.
  • Assist the client in problem-solving to help resolve any questions as they arise.
  • Maintain accurate documentation of client education in the electronic medical record.

Education and/or Experience

Must be at least 18 years of age, is a high school graduate or equivalent, and has at least one year or more of full time paid practical experience in providing perinatal care.

Language Skills

Ability to read and interpret health education materials for patients. Ability to write and document patient education and correspondence. Ability to speak effectively with individual clients, groups of customers or employees of the organization. Bilingual in Spanish is not required, but preferred.

Certificates, Licenses, Registrations

Possession of current, valid, California Driver’s License, annual health examination, annual TB skin test clearance or bi-annual chest x-ray, if required, MMR immunity and HBV optional, required.

Must be willing to have a flexible work schedule and travel to other clinic sites on an as needed basis. Will regularly travel to San Luis Obispo and Templeton every week.
